Dear Tenant,
As per the terms of your lease agreement, your rent is due every month on the {date}. If the rent is not paid by the {date} each month, a late fee of {amount} is automatically added to the total.
At this point, your rent for the period of {dates} is now {number} days late. The total owed is {amount}, included the late fee.
You are hereby notified that you have until {date}, five days from the date of this letter, to either pay the rent in full (including the late fee), or else deliver possession of the premises to {landlord/manager}. Should you either refuse to pay or remain on the premises, {landlord/manager} will begin legal preceedings against you.
This is a legally binding document, a copy of which will remain in your file.
